W2 Information Form
2012 W2s for active employees were sent to the employee's home store. W2 forms for terminated or otherwise inactive employees were mailed standard first class mail to each employee's last known address on file with our payroll department.
If your address has changed since your last paycheck please complete the form below. If you are requesting a duplicate or reprinted W2, please complete the form below and indicate in the comments section that you are requesting a reprinted W2. We will begin completing W2 reprint requests February 15, 2013. Duplicate copies will be re-issued within 48 hours of receiving the request. If corrections to the original W2 are necessary, the corrected W2 form will be reissued within 5 business days.
